Kava in traditional medicine

Kava (Piper methysticum) has been cultivated across Oceania for more than two thousand years. In Hawai'i, Vanuatu, Fiji, Tonga, and Samoa the root is pounded and steeped in water, then shared from a communal bowl — to open ceremony, to settle disputes, to mark hospitality.

Different strains have long been recognised as having distinct characters, and growers select among them deliberately. Blending them is its own traditional practice: the goal is a rounder cup than any single strain gives on its own.

The pathways: what the research examines

Kavalactones are the compounds most associated with kava's character. Pharmacological research has focused on several pathways:

GABA-A receptor modulation. Kavalactones appear to act as positive allosteric modulators at GABA-A receptors, binding at a site distinct from the benzodiazepine site.
Voltage-gated ion channels. Several kavalactones show activity at voltage-gated sodium and calcium channels.
Monoamine oxidase B. Some kavalactones show reversible MAO-B inhibition in laboratory conditions.
Strain profile. Kava strains differ in the ratio of the six major kavalactones, which is the working explanation for why growers describe them so differently.
